Kouper, a new health tech startup focused on improving hospital discharge workflows, has officially launched with $10 million in seed funding. Backed by General Catalyst, 25Madison, and CVS Health Ventures, Kouper aims to solve one of healthcare’s most expensive and overlooked problems—patient transitions after leaving the hospital.
Founded by Salman Ali (previously of GetSnooze) and Ablimit Keskin, PhD (formerly of Komodo Health), the Kouper health tech startup is building a platform to track patients in real time after discharge. Whether from inpatient care, the ER, or urgent care, Kouper’s AI-powered system keeps patients engaged, identifies risk factors, and helps schedule follow-up appointments—without adding burdens to clinical staff.
The Costly Problem Kouper Is Trying to Solve
Hospital discharges are one of the most fragile points in the care journey. Missed follow-ups, patient confusion, and lack of coordination contribute to over $53 billion in avoidable hospital readmissions annually in the U.S.
Kouper’s platform integrates directly with hospitals to give care teams a full, real-time view of patients after they leave. Instead of juggling spreadsheets or outdated systems, Kouper brings everything into one place—so care decisions are faster, safer, and more personalized.
“Our entire focus is on building a better patient transition experience,” said Salman Ali, co-founder and CEO. “We’ve lived through the problem and spent years working with partners to design the system from the ground up. With growing support from health systems, we’re scaling fast.”
How Kouper Works and Why It’s Gaining Traction
Unlike traditional software rollouts, Kouper’s platform can be implemented in under a month. It doesn’t require large onboarding teams, making it accessible even to resource-constrained hospitals. Once live, the platform automates scheduling, flags risks, and engages patients via smart, AI-guided tools.
Some of Kouper’s early health system partners report impressive results:
- Over 50% increase in patient engagement
- More than 60% improvement in follow-up appointment rates
It’s not just another software tool—Kouper is positioning itself as the infrastructure for care handoffs in U.S. health systems.
General Catalyst, known for its Health Assurance thesis, led the round. “Kouper is solving one of healthcare’s most critical operational failures,” said Chris Bischoff, Managing Director. “They bring continuity, accountability, and outcomes to a space that has been fragmented for decades.”
25Madison, Kouper’s incubation partner, continues to support the company’s vision. “Salman and Ablimit’s deep empathy is baked into every feature,” said Merrill Anovick, General Partner. “With new payment models like TEAM emerging, it’s more important than ever for systems to adopt Kouper’s solution at scale.”
Early Adoption by Major Health Systems
Kouper is already working with major U.S. health systems, including:
- LifePoint Health, which has expanded Kouper’s footprint across multiple hospitals
- Henry Ford Health’s Mosaic Clinically Integrated Network, serving 550,000 lives
- Ascension Saint Thomas, currently piloting Kouper in Nashville
“We’ve seen Kouper take a systems-level approach from day one,” said Chris Frost, MD, Chief Medical Officer at LifePoint. “They’ve earned our trust.”
Courtland Keteyian, MD, at Henry Ford Health, added: “Our Care Hub teams now have access to powerful analytics and AI-backed tools. It’s a game changer.”
With funding secured and validation from leading hospital groups, Kouper health tech startup is scaling rapidly. The company is growing its engineering and clinical partnerships teams, expanding analytics capabilities, and deepening integrations with early adopters.
Kouper’s long-term vision is clear: to become the default platform for AI-powered transitions of care. By building the digital “plumbing” for seamless handoffs between hospitals and the home, Kouper hopes to improve outcomes, reduce readmissions, and empower overburdened care teams.
